Required Documents for Long-Term Foreign Experts to Apply for Work-Type Residence Permit

1.护照或其他国际旅行证件、签证页、入境验讫章原件和复印件；

Originals and photo copies of valid passport or other international travel documents, visa, entry stamp;

2.《外国人签证证件申请表》（申请现场填写）；

Visa, Stay Permit and Residence Permit Application Form(Fill in on the application spot);

3.住宿登记证明原件；

Original ofAccommodation Registration Form for Foreign Nationals;

4.《外国人工作许可证》原件；

Original ofForeigner’s Work Permit;

5.工作单位证明函件原件（国际交流合作处提供）；

Original of work certificate (Issued by International Office of Exchange and Cooperation);

6.《境外人员体格检查记录验证证明》原件（6个月内有效且只有申请1年以上工作类居留许可需要）；

Original ofCertificate of Verification for Physical Examination Record for Foreigners(Within 6 months and only work-type residence permits valid for more than 1 year need to be provided);

7.涉外单位联系卡（国际交流合作处提供）；

Contact card (Issued by International Office of Exchange and Cooperation);

8.《行政许可决定书》原件和复印件（外国专家局提供）；

Original and photo copy ofDecision Letter of Administrative license(Issued by Administration of Foreign Experts Affairs);

9.2寸白底证件照（6个月内）。
