Required Documents for Long-Term Foreign Experts to Apply for theNotification Letter of Foreigner’s Work Permit in the People’s Republic of China

1.《外国人来华工作许可申请表》：原件和电子版（系统填报申请人信息后自动生成，需申请人签字，国际交流合作处（后简称“国合处”）盖章）；

Application Form for Foreigner’s Work Permit: original and electronic version(Automatically generated after filling in applicant’s information in the system, signature of applicant is needed, stamp of International Office of Exchange and Cooperation (IOEC) is needed);

2.申请人工作资历证明：原件和电子版，国合处盖章的中文翻译件（由申请人原工作过的单位出具从事与现聘用岗位工作相关的工作经历证明，包括职位、工作时间或曾经做过的项目，需申请人原工作单位加盖公章或负责人签字，并留有证明联系人有效联系电话或电子邮件）；

Work experience certificate of applicant: original and electronic version, Chinese translation version sealed by IOEC(The certificate of work experience related to the current employment position issued by the unit where the applicant used to work, including the position, working hours or projects that have been done, the official seal of the applicant's original work unit or the signature of the person in charge are required, and the effective telephone or email of contact person shall be on the certificate);

3.最高学位（学历）证书：原件和电子版，国合处盖章的中文翻译件（如境外获得），官方承认的学历认证机构开具的学历认证（如境外获得）；

Certificate of the highest degree:originaland electronicversion,Chinese translation version sealed by IOEC(If obtained overseas),academicattestation byembassy,consulate, orofficiallyrecognizedcertificationbody(If obtained overseas);

4.无犯罪记录证明：原件和电子版，外国高端人才（A类）可采取承诺制（应当由申请人国籍国或经常居住地警察、安全、法院等部门出具并经我驻外使、领馆认证或外国驻华使、领馆认证，承诺书模板见附件）；

Certificate of no criminal record: original and electronic version, high level foreign talents (Category A) can promise themselves no criminal records(It shall be issued by the police, security, court and other departments of the applicant's country of nationality or place of habitual residence as well as certified by Chinese embassy or consulate abroad or by foreign embassy or consulate in China, the commitment letter template is attached);

5.体检证明：原件和电子版（由中国检验检疫局出具的《境外人员体格检查记录验证证明》或健康检查证明书，或6个月内经中国检验检疫局认可的境外卫生医疗机构出具的体检证明）；

Physical examination report: original and electronic version(Certificate of Verification for Physical Examination Record for Foreignersissued by Entry-Exit Inspection and Quarantine Bureau of China, or physical examination report within 6 months issued by overseas health and medical institutions approved by Entry-Exit Inspection and Quarantine Bureau of China);

6.聘用合同：原件和电子版；

Employment contract: original and electronic version;

7.有效护照：原件和电子版；

Valid passport: original and electronic version;

8.正面免冠证件照：原件和电子版（6个月内）；

Photo: original and electronic version(Within 6 months);

9.随行家属相关证明材料：原件和电子版（包括护照、家属关系证明、18岁以上家属需提供体检证明、证件照）。

Relevant certificates of accompanying family member: original and electronic version(Including passport, proof of family relationship, physical examination report is required to provide by family member over 18, photo).
